Are these fixed plates or are they adjustable? Is there any stack height?

Fully adjustable. I was running -2.5 degrees camber with them. They are solid and hold the alignment very well.
These were paired with a Bilstein Sport / H&R race setup which uses the stock upper spring plate.

Can they add positive camber to a car that's been lowered?

you can, these rotate ~360 degrees to allow adjustment in every direction. For this price it's also an amazing deal.

These are the best street camber plates period. If I didn't have these already on my car I would have bought them already.

Alright, so I finally decided to put these on. But how the hell do you rotate them 180 degrees? The stamping says to 'disassemble and rotate bush'. OK. So I hammer out the studs and go to spin the bushing 180. Except now those 3 flanges coming off the bushing block the stud holes, because of course, they are not symmetrical. How can you rotate the bushing 180 degrees and still put it together?
