If your door cards don't have holes you must have a late model (88+) car or have newer door cards. If you have a late model car you have to change out the glass (and maybe the slide rails) to early model ones to use this set-up.

I have early model (4/87 build date) with late model cards. Question remains, would I just punch a hole in the card or is there more to it?
So tempted but my door cards don't have holes. Would I just punch it out? How do the handles afix to the crank?

You could make your own hole, the little nub that the crank handle attaches too just needs to be able to poke through. The OEM handle attaches to the crank mechanism with a bolt.
