I have the same problem. Here is my list of things I have done and I still have not found the cause. Some fixes are just routine maintenence. My car has just over 140K, I dont believe it is mileage/age related though.

*I recently replaced the valve breather hose (goes from valve cover to the ICV area), and the hose that goes from the fuel pressure regulator to the intake manifold.
I still idle at 600ish. I have heard about changing the coolant temp sensor. Any other ideas are greatly appriciated.
