early bumper misaligned/crooked? what causes this -- how to fix?

What causes this? I believe this was an issue when I first bought the car -- see how the bumper is slightly slanted down and to the right?

I have a few questions.
- Is this an issue with the frame?
- What can be fixed here?
- Is it an issue with the plastic bumper trim?
- Have any of you seen this before?
