How do I get this fastener in the center of the picture off?

Twist so that it passes through. Look on the other side - there is an index notch. I keep having to relearn that turning AWAY from the stopper is much easier. It's just a 90 degree turn.2006 GMC Sierra 2500HD 4WD LBZ/Allison
