Radio Code Issue?

Does anyone's radio ask for a radio code everyone you start the car? Could it be some wiring or fuses that connects to the radio being disconnected every time I turn my car off?

There should be +12V on the yellow wire all the time (even with the key off). If there isn't the radio will loose it's memory.
