Do you have any pictures of your setup?

When you first start the car it enters the warm up phase and ignores all of the adjustments that you made to the AFM wheel (if you made any). This could be one of your issues.

After the car is warmed up, how does it idle?

              Looks like a nice setup.

When you first put in the chip, did you adjust the afm wheel at all? It's the little wheel under the AFM cover on the intake side.

You can wrap the stock AFM boot in fiberglass if you want to make it stronger (I've seen tape and zip ties work as well). I think those things fail at ~8 lbs of boost depending on your turbo.

I'm glad the AFM wheel helped. How many clicks did you go over? Do you have an RRFPR/wideband?

I'd highly suggest a silicone air intake boot. If only there was a company who made one.

Everyone is gonna tell you that MS2 is better. Whodwho builds boxes that allows you car to be running in 30 minutes after installation.
