Clutch/transmission rattle/shake

So my clutch is semi jerky and hard to engage smoothly, and sometimes the car makes a rattling sound on start+when letting the clutch out in first gear.

Warped pressure plate?
Shitty throw out bearing?
Worn clutch?
Bad slave cylinder?

Anyone have this issue before? I haven't touched a clutch job yet and hope it's something simple like the slave.

That sounds more like problem inside the bell housing than a slave cylinder.
