car lost heat

My car's heater doesn't work anymore. I noticed that the temp gauge reads slightly to the left of the middle and at the same time there's condensation all over my windows?

The engine temperature sounds normal and the condensation on the windows may not have anything to do with the lack of heat. As a guess I'd suspect a blown fuse, bad heater control or valve, or a plugged heater core.
