Code 1231 + High Battery Voltage

So yesterday my check engine light started flashing occasionally while driving. I pulled the code and it showed 1231 (Battery Voltage/DME). With the car off my battery voltage is 12.8V and with the car running, its right around 16V! I pulled the alternator to have it tested at AutoZone and it passed. I also inspected the voltage regulator brushes and they look okay. Aside from the check engine light, the car runs okay. Any ideas?

The voltage regulator in the alternator is bad. Max system voltage should be close to 14.5v.
The car makes it possible, but the driver makes it happen.

New alternator fixed it! Don't trust AutoZone alternator testers. Thanks, Jim.

They probably tested if it was not making enough, not too much.
