Rear subframe bushings slipping out of subframe

Has anyone seen this problem before, installed a new set of rear subframe bushings into my e30 this weekend and drove it around and found out the subframe on the driver side has slipped up the bushing.
My question is has anyone had this problem before and is there other posts about this happening? defective bushing maybe? or just happens from the torque of the engine?

    thanks

What material are the bushings? OEM Rubber/Poly?

Might you have left off the large washer that goes between the subframe and body?
The car makes it possible, but the driver makes it happen.

"Slipped up the bushing" ? I'm having a hard time picturing this. I don't suppose a picture could be taken and posted? We might be able to identify if anything is missing that should be there :)

Sounds like something was done incorrectly. Not seeing how this could happen...
