Sorry to hear about your misfortune. I had the same thing happen to me about 3 months ago. A girl rear-ended me in a newer Ford Explorer going about 30mph. The damage wasn't quite as extensive as what your's is, she did try to lessen the impact, she just saw me too late. The Explorer was undrivable.
We realized we both had Progressive and instead of calling the police and sitting on the side of the road all day, I suggested we just call the insurance together. She told them what happened, they took both of our policy numbers and had a appraiser out the next day.
I think because we both had Progressive they went out of their way to be fair with me. I told the appraiser from the beginning that he was not going to total the car (it's a 90 but has 230,000 miles.) He basically he offered me a check then for what he thought the damage might be worth, around 1200, or I could put it in the shop and they would pay the repair bill no matter the cost. I chose option B since I knew to be fixed right would be more than 1200 and in the end it ended up being 2200, not including the rental car I had for over a week that they also paid for. They even fixed a pre-existing ding in my quarter.
I hope you have a similar result. In this economy I think they may do a better job than standard to keep you. The other guys insurance is going to go up and they don't want to risk losing two customers.
If it is totalled tell them to give you 4500 for it. Seems to be the going rate these days for 'clukers'

I'll say get a lawyer! even if you have the same insurance, and yes see a doctor. it might not hurt much now but once you get older you will feel it more if you never got treatment for the neck/back. I just got hit las saturday and the ladys insurance is 21st Century. I got a lawyer and a doctor. waiting for the insurance to check the car but mine is hit bad! 100% they will totaled,
