Please help!! Won't pass!! Smog -__-

The last one was today, the best one was a month ago.. I've driven the car a lot. About 1500 miles already.

Fresh oil and oil filter (after first smog)
O2 sensor from my iS(passed smog flying colors) (after 3rd smog)
    Fresh 91 all the time.
"Guaranteed pass smog" crap. (After 3rd smog)
Seafoam in the gas tank (after 1st, 2nd, and 3rd smog)
Changed header-downpipe exhaust flanges or gaskets whatever they're called. (After 3rd smog)

Does it need a new cat? Also, getting the car super hot, even letting it idle while you're filling out the paperwork, supposedly helps.

Those numbers would have me running a smoke test and fixing intake leaks, replacing the distributor and ignition wires, checking and possibly replacing the ECT sensor, making sure that the engine is running at normal temperature (80C), and adjusting the valves. That should get the HC and CO2 numbers back where they should be. If NOX is still too high, replace the catalytic converters.
The car makes it possible, but the driver makes it happen.

Forgot to mention new thermostat, but haven't bled the system. Doesn't overheat though..

For anyone that knows about this stuff and is trying to help me, idk if this will help, but as you notice the numbers get worse every time. (Besides the last time on 15mph) So I guess the more I drive, the worse it gets...
