There was sad story this reminds me of a women in china was backing out of
a tight parking spot with her husband outside spotting her.

I think she thought the gas was the brake but she completely smashed him
in between a concrete wall and the door. On top of that she had her head out the window
and crushed her head on the wall. Both died.

In regards to that Chinese story, iirc their daughter was in the backseat when that happened. Had forgotten about it till you mentioned it. That probably takes the cake as far as unfortunate, especially given the fact that it was accidental.
