i have done it with corded headphones/phone headset, just took out one speaker and replaced it with a headphone speaker so when on the phone i could also listen to music, i know my friend who works at a place where they all have headsets and walkytalkie's, has a headset with a little ear bud so he can listen to his ipod.
not sure it will work with bluetooth since it wireless/

i think this is more of a software issue then a hardware issue isn't it? I mean, the headset already has a speaker in it, so I don't see what replacing it is gonna change? It just depends on when the speaker is activated to output sound which would be controlled by the software. (IE - when u hit the button to make a call, or when you have an incoming call)

You basically want to make bluetooth headphones, right? Assuming that when the music is playing, it outputs through bluetooth to a headset, then it should work just fine.
There are a few places that sell wireless headphones however. And after buying a $70-$100 bluetooth headset and hacking it up, you might as well have just bought one of these. Some are bluetooth, some require you to plug a transmitter into the device. You might check them out.
