Need some HTML Coding Help, Web Gurus?

Hey guys, I need a little help with some HTML coding.

Basically building a form with a photo upload option and a couple radio buttons, but I can't get the email to work.

The form is finished and published, but when I submit there is no e-mail submitted. I tried the code a couple different ways but no luck yet.

I'm using Dreamweaver CS3, and hosting through Dot5.

I see nothing wrong with your front end code, but noticed you're bouncing it off your provider's mail form script. So there could be a conflict there, or they could be having issues. I'd contact them to trouble shoot, or look for a tutorial on a simple PHP form, that way you can control all variables.
