Turned out to be a bad ICV. Passed today with flying colors! Went from 2700 to 45 NOx, then single digits across the board with 0% CO. The DMV should be paying me for such a low emitting E30.
Things I did between first and second check for others in the future:
-replaced cat
-replaced O2 sensor
-replaced fuel filter
-replaced ICV
-coolant flush
-cleaned intake manifold (glass bead blasting)
-cleaned MAF
-replaced/repaired a couple old or cracked vacuum hoses
-seafoam in intake
