Oil, dust are no help to the surface, that's for sure - but that's pretty self-explanatory. Proper cleaning is important, but from the scenario that Colt is giving us, it appears that it is more than that.
If it was the surface dust, oil etc, the primer would not stick properly, and he probably would not go painting the final coat on.
Like Jordan said, the paint is not sticking to the primer, its the paint inmcompatibility (or should I say Paint/primer incompatibility. . . I thought this was also self-explanatory).
