In normal operation, the top rad hose is hot and the lower one is cold (it is the outlet from the rad, the cooled water flows in it). Your fan clutch may well be bad. I had one go out on me, and while I never overheated, it made nasty noises.
Again though, if you are getting constant gaseous expulsions, then the problem is likely to be a head crack or warpage. If the coolant was actually boiling, the temperature needle would be buried deep in the red on the gauge (which you need to fix).
When you say "steam coming out" do you mean that steam and air bubbles come out of the bleed screw, or that coolant comes out of the bleed screw and some mist/steam comes off of that?
