Slave cylinder/clutch fork failure. Possible causes?

91 325i. I just did an out of the car engine refresh and 5-speed swap. We got everything back together and running with no major issues. Everything properly installed, clutch and brakes properly bled. I took a couple of short test drives with no issues, everything ran and shifted fine. On my way home from the garage, as I was taking off from a stop, the clutch pedal got stuck half-way in, and I could not move the shifter. I put my toes under the pedal and pulled it out, then I heard a hollow-sounding clunk and the engine died. At this point I could move the shifter to neutral and the clutch pedal felt pretty firm, but was not engaging the clutch. A quick visual inspection revealed fluid from the bell housing, so I know the slave was blown. I started it in first gear and flat-shifted her on home. Got under the car for a more thorough inspection and discovered that the slave cylinder rod was stuck fully extended and the boot was torn. The clutch fork retainer spring had come disconnected on the top side and the clutch fork was rotated just slightly clockwise (when viewed from the slave cylinder opening.

Any suggestions as to what could have caused this issue? I plan on dropping the transmission to reset the clutch fork and retainer spring, then re-installing with a new slave cylinder. Is there anything else that is suspect, that I should pay particularly close attention to? I really don't want a repeat of this problem.

    Any help is greatly appreciated.

      Could be.

I am in the process of dropping the gearbox now. I plan on inspecting everything and replacing anything that is questionable. I'll post up my findings once I get it out.
