any idea on why this happened? Could the tip of the plug still be in the cylinder somewhere?

you should give it a compression test i had a spark plug fail like that on a jetski i built and it scored the cylinder pretty bad which caused the compression to drop and it made a rattling sound for a few seconds then had no powerComment
