I would leave if:

• cheating has occurred - no second chances.

• She gives me shit about hanging with my friends - like tries to control me.

• Get's pissed off if I buy myself something insignificant, but she spends $$$$ on b.s. stuff
• Either of us is unhappy with no solution in sight.
• Sex stops.
• She wants me to change who I am. - My thought is that I am who I am. Whoever wants to be with me shouldn't want me to change that.
• If in the first few months of the relationship there have been 3 major fights, or many smaller fights.

Maybe a few other reasons. Also depends on how long the relationship has lasted.

Add to this "drug use" as in hard drugs, not weed, or too much booze and you nailed it.

My GF of 7 years moves out tomorrow. We still love each other, we just can't find much common ground to stand on. Better to quit while nothing really bad has happened, while you can still be friends.
