the composition is well done as well is your usage of light.
If I were to change anything I would illuminate the knife a little more so the the viewer instantly knows whats going on. I personally thought it was like a gardening shovel or just a handle when i looked.

I didn't illuminate the knife so much as that would detract as the whole image and make you focus straight on the knife, you can see its outline and still see a little glean on the corner back edge of it which tells you, hey its a knife, and you just fill in the pieces and create a story line for the photo in your head.
Also seeing the girl lying there can give the impression that something is about to happen to her, which in your head would place the thing in my hand as a knife or another life threatening object.
It is not nessicarly an image you can really just glance at and understand it fully. You need to just take a moment and look at it fully(not directing it at you, just in general)
I agree with you that images should be focused on rather than glanced at and immediately interpreted, I was just hoping for further enunciation of the knife.
The tip of the knife blends into the shadow losing its definition. If you have a light box or free standing flash you might be able to work some soft light in to the edge of the knife...
