I had my drivers rear wheel fall off after a few runs through the mountains (dragon). This was on Kosei K1s, which are actually known for doing this (sanding the lug holes fixes it -- so it seems). Caused zero damage to any part of the car except for a bent shield behind the rotor (just pulled it off of the rotor).
Funny thing is, a few days before, I torqued them to exact spec with a 1/2" torque wrench.

Rumour was that he tried to be slick an reduce rotational mass by removing one lug nut on each wheel. I don't think that turned out well.

            That vid is ancient though....

autocross in atl last month, a dope infinity (FWIR) lost a wheel because the lugs stripped out. The shop the guy had the car mount the wheels cross threaded the lugs with an impact...

I'd never trust the monkeys at the local tire shops to do this right. The best part is, they won't touch my car anyway because it has aftermarket studs instead of factory lug bolts. :)

i had kosei's on one of my m3's with spacers and extended lugs, constantly had problems with the lugs coming out on one of the wheels. eventually fixed it thought, although never knew about that sanding trick.
