    HARD vibration from the rear...

Whenever I accelerate I get an incredible vibration from the rear between 18mph-22mph. This range will increase if I am accelerating harder or going up steep hills, and will not shake at all if I barely use any gas to get past that range. I'm thinking rear wheel bearings? I do have the wheel bearing hum so I know they need to be replaced.

check if your CV's have lost grease and are disassembling themselves, check that your diff's still attached, and check that your CSB is intact. I'd suspect all of those before the wheel bearings.

That sound most like a failed center support bearing and binding u-joints in the drive shaft. Replacement of the drive shaft and a new CSB is the fix.
The car makes it possible, but the driver makes it happen.

I don't think it'll be your wheel bearings unless you hear some whining as well (something like two metal plates spinning flat against each other).
I'm siding with jlevie on the CSB and the DS u-joints

I had a similar issue, immediately thought it was my CSB, ended up being that my exhaust was secured too tightly, and would rattle like crazy on the undercarriage at certian RPM's

Definitely not related to RPM in my case. This will happen in any gear and only at those speeds. I bought a new CSB just haven't installed it yet.
