drive around with windows down/sunroof back for a long time.
take the seats out for a day or so make sure they get aired out nice.
treat the carpet. treat the seats.

I have a 12v ionizer that I used on my last car before I quit. Worked alright.

multiple vigorous cleanings with strong chemical cleaners, and sadly, much time as a non smoking car.

also incorporate everyone elses suggestions, tobacco smoke is maybe the worst ever car smell. except for ripe dead hooker.
                          thats even worse.

You'll need to remove all the upper interior trim. andything plastic or rubber. You'll be shocked to see all the greasy shit. Head liner needs to come out as well. Grab an ozone generator and leav it in the car over night and for the next day.

Simple green everything that you took out the car and add some simple green to some shampoo and get to scrubbing the interior. This is pretty much the only way to completely get rid of the smell. I used to work with a company that restored cars and appliances and most of em belonged to smokers. This solution is what we trained our techs to do and we've never heard a single peep.

Pick up a dirty hooker one night and get busy in the car. That should trump the smell.
