I can't figure out how this happened. If one of the shells prematurely went off, why are there TWO blown out casings in there?

I mean, the thing rotates clockwise, makes sense that the one is missing a bullet if it's been fired, but then that makes even less sense that the casing is blown open!

I have a good theory, some one over loaded some hand loads and over pressured the cylinder. Or there was a serious barrel obstruction but the lack of evidence on the barrel leads me to think it was a over loaded round.

Reminds me of this failure WWII weapon. It had thrust rockets along the rim to get it going. There were explosives in the middle drum that was supposed to blow up when it got to a target. They could never get it to go where they intended though.
