Anyone can help me out with this on what to do? My uncle hit a pot hole and this happened. Looks like the driver side rear subframe broke from 2 bolts that hold it.

Ouch... I have some of the subframe bushing plates that you need but I would check out where it bolts up to make sure your still good there. Or you can get the plate at the junk yard which is most likely a little closer drive.
